The newly created post of Transitions Coordinator commencing in January 2020 will be based at our small and friendly North-West office based in Hazel Grove, Stockport.

You will be responsible for ensuring all children who live in/attend our schools & homes have an optimistic path forward in life, supporting them to make a positive contribution to, and be a valued member of, society. This could be through supporting them into further education, skills training/apprenticeships, employment or other valuable endeavours commensurate with their abilities/needs. 

Your role will see you ensuring systems exist and are maintained and monitored in each provision within Kedleston Group to support children onward from the organisation.  

Significant time will be given to working with on-site staff who are responsible for transitions, as well as with the children who are ready for transition, working with them to help support with planning for transitions before they reach school/home leaving age and supporting them through the actual transition process.

Additionally, you will provide support to children post transition and until they are individually self-sufficient and have built their own external support networks to succeed.   

To be successful in this role you will have demonstrable knowledge, experience and understanding of working with and supporting children with special emotional and mental health needs, as well as colleagues within education and care provisions, to plan and deliver successful transitions, which are based on individual needs.

You will be a strong communicator, adept at working collaboratively across teams to design, deliver, evaluate and monitor transition plans, actively seeking, recording and acting upon feedback to continually shape and improve the services we deliver.

Your role will require significant travel to visit colleagues, young people and children in our various provisions.
